Lorazepam (widely known globally as Ativan, and in India as Lopez or Larpose) is a highly potent, intermediate-acting benzodiazepine. In the treatment of Bipolar Disorder, it is not a mood stabilizer, nor is it an antipsychotic. It is the ultimate “rescue medication” used to quickly stop panic and manic agitation.
Section 1: What is this medicine & how it works
1. Drug Class & Category
- Primary Class: Benzodiazepine.
- Psychiatric Category: Anxiolytic / Sedative-Hypnotic.
2. Mechanism of Action (Simple Language)
When a patient is experiencing acute manic agitation or a severe panic attack, their brain’s electrical activity is racing out of control. Lorazepam acts directly on the brain’s “calm down” system (the GABA system). It forces the brain’s calming channels to open wider, instantly slowing down brain activity, shutting off panic, anxiety, and the manic “buzz.”
- Medical Detail: Lorazepam binds to an allosteric site on the GABA-A receptor. This enhances the affinity of the receptor for GABA (the primary inhibitory neurotransmitter). The increased GABA binding opens chloride ion channels, hyperpolarizing the neuron and rendering it less excitable.
3. Approved Medical Uses (Indications)
- Anxiety Disorders: Management of anxiety disorders or the short-term relief of the symptoms of anxiety or anxiety associated with depressive symptoms.
- Insomnia: Short-term treatment of insomnia.
- Preanesthetic Medication: To produce sedation, relieve anxiety, and decrease the ability to recall events related to the day of surgery.
- Status Epilepticus: Intravenous Lorazepam is the first-line treatment for continuous seizures.
4. Off-Label Uses
- Acute Manic Agitation: Used in Bipolar Disorder to calm highly volatile, aggressive patients until primary mood stabilizers take effect.
- Catatonia: First-line treatment for psychiatric catatonia (a state where the patient is frozen or mute).
- Alcohol Withdrawal: Used to prevent seizures during severe alcohol detox.
5. How Quickly it Starts Working
- Onset of Action: When taken orally, it begins to work within 20 to 30 minutes. An intravenous injection works almost instantaneously (1 to 5 minutes).
6. Half-Life and Duration of Effect
- The biological half-life is 10 to 20 hours. The clinical sedative effects generally last for 6 to 8 hours.
Section 2: Dosage, Forms & How to Take It
7. Standard Adult Dose & Frequency
- For Acute Anxiety/Panic: 0.5 mg to 1 mg taken two to three times a day as needed.
- For Severe Manic Agitation or Insomnia: 2 mg taken at night.
8. Available Forms
- Standard Oral Tablets: 0.5mg, 1mg, 2mg.
- Intravenous/Intramuscular (IV/IM) Injection: 2mg/mL or 4mg/mL (Used exclusively in hospitals for immediate sedation or stopping seizures).
9. How & When to Take It
- With or Without Food: Can be taken with or without food.
- Time of Day: Usually taken strictly on an “as-needed” basis for panic, or at bedtime for insomnia and manic agitation.
10. What to do if you miss a dose
- Since it is usually taken “as-needed,” there is no issue with missing a dose. If you are on a strict daily schedule (which is rare), take it as soon as you remember. Never double the dose to “catch up.”
11. Maximum Daily Dose
- The maximum daily dose rarely exceeds 6 mg to 10 mg in severe psychiatric inpatient settings. Outpatient doses should be kept as low as possible.
12. How to Store the Medicine
- Store at room temperature (20° to 25°C). Keep heavily protected from light and moisture. Keep strictly out of reach of children or anyone with a history of substance abuse.
