Mental health is arguably the most neglected area of Indian healthcare. But the government has built a surprisingly extensive (though imperfect) free mental health system. Here is everything you need to know.

1. Tele-MANAS (Tele Mental Health Assistance & Networking Across States)
This is India’s national tele-mental health helpline, launched in 2022.
- Free, 24/7, available in all major Indian languages.
- Provides immediate counseling over the phone.
- Can refer you to the nearest government mental health facility.
- Trained counselors and psychiatrists available.
- No registration required. Just call.
2. District Mental Health Programme (DMHP)
The DMHP is the backbone of India’s public mental health system. It operates clinics at District Hospitals, CHCs, and PHCs across the country.
What DMHP provides (FREE):
- Psychiatric OPD consultation
- Essential psychiatric medicines (antidepressants, mood stabilizers, antipsychotics, anti-anxiety)
- Counseling and psychotherapy
- Follow-up care
- Referral to higher centres for complex cases
How to access:
- Visit your nearest District Hospital or Community Health Centre (CHC).
- Ask for the DMHP clinic or Psychiatry department.
- Carry your Aadhaar card.
- Consultation and basic medicines are free.
3. Top Government Mental Health Institutions
NIMHANS (National Institute of Mental Health & Neuro Sciences), Bengaluru
- India’s #1 mental health institution. Established 1974.
- OPD: ₹10 registration.
- Departments: Adult Psychiatry, Child Psychiatry, Clinical Psychology, Neurology, Neurosurgery, De-addiction, Psychopharmacology.
- Best for: Treatment-resistant depression, bipolar disorder, schizophrenia, epilepsy, substance abuse.

Central Institute of Psychiatry (CIP), Ranchi
- India’s #2 mental health institution.
- OPD: Nominal charges.
- Best for: Psychosis, severe bipolar disorder, forensic psychiatry.

- Free psychiatric services.
AIIMS Psychiatry Departments (All AIIMS Campuses)
- Every AIIMS has a Department of Psychiatry.
- OPD: ₹10. Medicines: subsidized.
4. Specific Conditions — Where to Go
Depression:
- Mild/Moderate: Start at your nearest DMHP clinic or PHC. Free counseling + SSRIs (Fluoxetine, Sertraline).
- Severe/Treatment-Resistant: NIMHANS, CIP Ranchi, or AIIMS Psychiatry.
Bipolar Disorder:
- Mood stabilizers (Lithium, Valproate, Carbamazepine) are available free at DMHP clinics.
- For complex cases: NIMHANS Bengaluru is the gold standard.
- Regular monitoring (Lithium levels, thyroid): Free at government hospital labs.
Anxiety Disorders (GAD, Panic, Social Anxiety, OCD):
- DMHP clinics provide free SSRIs and counseling.
- Severe cases: NIMHANS or AIIMS Psychiatry OPD.
Schizophrenia:
- All government psychiatric hospitals provide free or subsidized long-term care.
- India has 47 government mental hospitals for chronic psychiatric patients.
5. Your Rights Under the Mental Healthcare Act, 2017
This is one of India’s most progressive laws:
- Right to free treatment at government-run mental health facilities.
- Right to community living — You cannot be institutionalized against your will without proper legal process.
- Right to legal aid — Free legal representation for persons with mental illness.
- Insurance must cover mental health — All health insurance policies must cover mental health treatment on par with physical illness.
- Attempted suicide is decriminalized — You cannot be prosecuted for attempting suicide.
